Metroplex Medical Transportation (MMT) offers safe, reliable wheelchair transportation services for healthcare facilities in Ft. Worth metropolitan area. We recognize the need to have a service that can provide on time accessible transportation for medical appointments, hospital discharges and urgent treatment or procedures.
How do you get started?
Step One:
Download and complete the Billing Authorization Form that gives us the facility’s primary contact name, your billing address and signatures from the staff members that are authorized to request private pay transportation.
Fax the form to MMT at (682) 410-0166 and it will be processed in 1 to 3 business days. You will receive an email confirmation stating that the form was received and accepted.
Step Two:
You can submit a trip request through the website by selecting Request A Quote or download and complete the Transportation Request Form and fax it to MMT at (682) 410-0166. You will receive the confirmation by fax or email.
- A trip request forms (online or by fax) should be submitted 48 hours before the scheduled appointment or the schedules may already be full. All requests must be received no later than 3:00 pm the day before the scheduled appointment – but it is schedule to as soon as possible.
- Same day requests (we will check availability) should be called in to Dispatch at (682) 410-0112. Please note – there may be an additional fee for same day trips.
Please fax in your billing authorization form and your facility will be registered in our system. You can begin scheduling trips when you receive your confirmation fax verifying that we have all your information. It is that easy!
Our drivers will provide door to door assistance and are trained in working with individuals that may require additional assistance.
